North suburban skilled nursing facility is seeking a Physical Therapist (PT) to provide vacation coverage from 7/9/12-7/20/12.  This PT will be responsible for providing evaluation and treatment to patients for both short term rehabilitation and long term care.  Prior SNF experience is required.

Valid Illinois Physical Therapy license required, CPR required, References required, Adherence to all Physical Therapy rules and regulations set forth by the Illinois Department of Professional Regulation required.
