Physical Therapist (PT) needed to see 3 therapy patients for home health Physical Therapy services on Chicago's southside.  All of the patients are seen for PT services 2 times per week.  The Physical Therapist is responsible for providing evaluation and establishing a treatment plan that increases each patient's mobility and safety in their home environment and community.  This position is available immediately.

Illinois Physical Therapy license required, CPR required, References required, Adherence to all Physical Therapy rules and regulations set forth by the Illinois Department of Professional Regulations.
